Request a Change of Address
To change your address for all or some of your bank records, download and print the "Official Notification of Address Change" form. Once you have printed the form, please fill in all information, sign and date it and either mail the form to the Community Bank Main Branch (101 Community Blvd Longview, TX 75605) or deliver it to any Community Bank branch. Please note, to protect your account information, this form requires an original signature, and cannot be faxed or emailed.
